Substandard conditions, or substandard acts?
Below are two examples of a Tripod Beta tree for an incident where a pipe leaked due to corrosion. In the first version, immediate causes can be classified as 'substandard conditions' (i.e. corrosion).
However, in the second example, substandard conditions are not used, and immediate causes instead need to be classified as substandard acts - i.e. human failures. The substandard condition is remodelled as a Tripod trio and the human failures that led to the corrosion, both at design stage and inspection and maintenence stage, are identified.
